Care
SOCH has a proactive policy designed to ensure the proper identification
of patients prior to performing any and all tests, administering
any and all medications, sampling blood for laboratory tests, administering
blood or blood components, and transporting a patient to any area
for testing or procedure.
To ensure proper identification, staff
will ask the patient for their name and birthdate, check the
patient’s wristband and
make sure the information matches what is on the medication label
and on the patient’s electronic medication administration
record.
The staff at SOCH makes sure that patients
know that these steps are taken to ensure their safety. Patients should know that
everyone coming in contact with them should ask for their name
and date of birth. Patients are encouraged to remind caregivers
to do this if they neglect to check this vital information.
