Bring the List
Like most people, you probably have more than one doctor caring
for you. You may be taking medications from different doctors.
It is important that ALL doctors and nurses know ALL the medications
you are taking. It is also important to have a list of all of your
medications in an emergency. When doctors have a list of the medications
you are taking, they can give you the best care.
Step 1. Make a list of all of your medications

2. Pick up the "Bring the List" brochure which includes
a wallet-size medication record. Brochures are available in the
main lobby of SOCH or at the SOCH Family Resource Center
Simply fill out the form listing of all your medications and your
dosage. Be sure to add new medications and cross out those you
no longer take to keep your record up to date. Your doctor or pharmacist
can help you if you have questions.
Step 2. "Bring the List"
Bring your list of medications to all of your doctors and healthcare
appointments. This makes it easy for you when you are asked what
medications you are taking. It is also a good idea to let your
loved ones know where you keep this list in case of an emergency.
Or better yet, keep it with you in your purse or wallet at all
times.
For caregivers and family members taking care of a loved one,
keeping a medication list for your family member will help you
with their medical care.
Step 3. Update your list
Keep your list up to date by adding any new medications your doctor
(s) wants you to take and crossing off any medications on your
list that you have been told to stop. To be most helpful, your
list should be up to date and correct. It takes a few minutes,
but the value it brings to your healthcare is well worth the time. |
